Please attach photos or pdfs of your receipts to this form. Unfortunately the PTA can't reimburse sales tax so please deduct the sales tax from your receipt's total amount. 

Attach up to 10 files, note that the attachments' total size must be less than 15 MB.

The PTA treasurer will review the receipts and reimburse you via check.
